Documentation requirements scale proportionally to risk
The organization applies documentation and approval burdens in proportion to the actual risk or dollar value of the action being taken. Low-risk, low-value items do not require the same compliance overhead as high-risk decisions. Blanket documentation rules that ignore risk level are treated as a design failure.
